In a medium saucepan, mix the cream cheese, flour and garlic until well blended. Stir in the milk. Bring to a boil over high heat, whisking constantly. Reduce the heat to medium-low and simmer for 2 minutes, or until thickened. Add the Parmesan and pepper; mix well. Remove from the heat.

Cook the fettuccine in a large pot of boiling water for 2 minutes, or until tender but still firm to the bite. Drain thoroughly in a colander and return to the pot. Add the sauce and toss to mix well. Serve sprinkled with the parsley (if using).
